Since 2002, Disease Management Programs (DMPs) have been established in Germany. They include regular doctor visits, detailed consultations, and follow-up examinations. A key focus is on improving cooperation between specialists and healthcare facilities to ensure the best possible treatment coordination.
An important part of managing COPD and asthma is regular lung function testing, such as spirometry. These tests show how well your lungs are working by measuring lung volumes and airflow. This helps us monitor your condition and adjust your treatment accordingly.
As a DMP participant, you benefit from personalized care by our doctors and medical team. We closely monitor your health to detect and prevent complications early. Together, we work on optimizing your treatment and improving your quality of life.
How can you join a DMP?
If you have diabetes, COPD/asthma, or coronary heart disease (CHD), you are welcome to contact us. To take part, you need a confirmed diagnosis and be insured with a public health insurance provider. Participation in the DMP is free of charge. Please bring any existing medical records to your first appointment. We will then discuss whether the DMP is suitable for you and what steps are needed.
